I need an online "File Registry System" based on PHP and mySQL.

The File Registry System will basically work like a Library Loan/Borrow System, and by File = Physical Hardcopy Files.

Some basic requirements:
- Create new file records into database with specified fields
- Create new file volume (based on file records) into database
- Closure of files
- Send automated email reminders when file mature in the system and require revision
- User database (Normal user: only search records; Registry officers: search records & facilitate for loan/return of files; Admin officers: search, borrow/return and also create new file records or close records)
- Data capture for all records movements, fields updating, including loan histories
- Generation of reports: Loan report, File creation report, Volume creation report, File Closing report, File transfer report, Retention report, etc.
- Other ad-hoc requests by my client
